Her situation highlights a nuanced area of blood health, where abnormal cells are present but have not yet caused the significant health issues associated with full-blown myeloma.
Smouldering myeloma is characterised by the presence of a 'clone' of abnormal plasma cells within the bone marrow. Plasma cells are a type of white blood cell that normally produce antibodies to fight infection. In smouldering myeloma, a single rogue plasma cell multiplies, producing identical copies and a specific 'monoclonal' protein measurable in the blood. Crucially, at this stage, there is no organ damage, blood counts remain normal, kidney function is preserved, and bones are intact, meaning individuals typically feel well.
The condition sits between a harmless, low-level presence of monoclonal protein often seen in older adults and symptomatic myeloma, where the abnormal cells actively harm the body, leading to anaemia, bone pain, fatigue, or infections. For patients diagnosed with smouldering myeloma, the absence of immediate treatment can feel counterintuitive, but it reflects the nature of the condition as a 'halfway house' before the onset of more severe symptoms.
Not all individuals diagnosed with smouldering myeloma will progress to symptomatic myeloma. While some may see their condition advance over a few years, others can remain stable for a decade or longer without needing intervention. Doctors monitor the condition closely, using blood tests, bone marrow findings, and sometimes genetic markers to estimate the risk of progression. Factors such as higher protein levels or extensive bone marrow involvement can increase this risk.
According to NHS guidelines, early detection and regular monitoring are key for conditions like smouldering myeloma. NICE recommendations often guide the management protocols for such conditions, ensuring that patients receive appropriate care based on the latest evidence. While the overall incidence of myeloma is relatively low in the UK, affecting around 6,000 people annually, understanding its early stages like smouldering myeloma is vital for early intervention if progression occurs.
