Existing exercise guidance for individuals suffering from Long Covid may be inadvertently causing more harm than good, according to new research from the University of Oxford. The study highlights that many current recommendations fail to adequately address post-exertional malaise (PEM), a debilitating symptom characterised by a significant worsening of symptoms after even minimal physical or mental exertion.
PEM is a hallmark symptom for many Long Covid patients, yet general exercise advice, often promoting gradual increases in activity, may be exacerbating their condition. Researchers at Oxford, led by Professor Sarah Walker, emphasised that while exercise can be beneficial for general health, its application in Long Covid must be carefully considered, particularly for those experiencing PEM. The findings suggest that a 'one-size-fits-all' approach to exercise is inappropriate and potentially detrimental for this patient group.
The study, which has been peer-reviewed, critically examines the evidence base for exercise interventions in Long Covid. It notes that much of the previous research has not sufficiently accounted for the unique physiological responses of individuals with PEM. Instead, it has often drawn on evidence from other conditions where exercise is broadly beneficial, without considering the specific pathology of Long Covid.
This research builds upon a growing body of evidence indicating the complexity of Long Covid and the need for highly individualised care plans. Earlier studies have also pointed to the varied symptomology of Long Covid, which can include fatigue, breathlessness, cognitive dysfunction, and a range of other issues, making a standardised approach to treatment challenging. The Oxford team's work underscores the importance of symptom-specific management strategies.
The practical implications for UK society are significant. With an estimated 2 million people in the UK experiencing Long Covid, according to the Office for National Statistics, a re-evaluation of clinical guidelines for exercise is crucial. Healthcare professionals, including GPs and physiotherapists, may need to adapt their advice to ensure it does not worsen patients' conditions, especially those with PEM. This could involve promoting rest and pacing strategies over graded exercise therapy for certain individuals.
The researchers advocate for a more cautious and personalised approach, potentially focusing on symptom management and energy conservation rather than conventional exercise programmes for patients exhibiting PEM. This shift could prevent further deterioration in health for many Long Covid sufferers and improve their quality of life.
