The mandate issued to NHS England and the accompanying 2025/26 Operational Planning Guidance outline a requirement for the health service to achieve 2.5% annual productivity growth. This target is considerably higher than the average 1.5% annual growth seen over the past decade and the 0.9% average over the last 40 years. The Nuffield Trust highlights that achieving such a rapid increase in efficiency without substantial capital investment or a significant shift in operational models will be extremely difficult.
The guidance sets out key priorities for NHS organisations, including continuing to reduce waiting lists for elective care, improving urgent and emergency care performance, and enhancing access to primary care services. These objectives come against a backdrop of persistent pressures, including an ageing population, rising demand, and the ongoing impact of the COVID-19 pandemic on service delivery and staff morale.
Experts at the Nuffield Trust argue that while efficiency drives are necessary, the current targets may be unattainable, potentially leading to further strain on services or a compromise in the quality of care. They emphasise that sustainable productivity improvements often require upfront capital investment in new technology, infrastructure, and workforce development, which has not been consistently forthcoming.
