Authorities in Slough failed to adequately protect a 19-year-old who was fatally stabbed, despite his repeated pleas to be moved to safety, a new report has revealed. Temur Qureshi died in September 2023 after being attacked by Jack Patterson in the town. An inquiry by the Slough Safeguarding Partnership concluded there were "missed opportunities" to intervene and safeguard Qureshi.
The report details how Qureshi had expressed profound fears for his life if he remained in Slough, telling investigators, "He was living in fear and no-one was listening to him. He was very afraid that he would be killed, and he told many people this... He wanted a place outside of Slough as he did not feel safe here." A youth worker attempted to facilitate a move for Qureshi, but this was reportedly rejected by the local council's housing department.
Qureshi's vulnerability was heightened by a severe brain injury sustained in a brutal attack at Feltham Young Offender Institution in July 2022, which left him forgetful and requiring constant care. Furthermore, he was grappling with the loss of his best friend, Abdul Aziz Ansari, who had also been fatally stabbed just two months prior to Qureshi's own death. Reviewers noted that Qureshi "was not considered as a victim or as an adult needing safeguarding although all partners now agree that he was both."
The inquiry identified several instances where authorities could have formed more constructive relationships with both Qureshi and his attacker, Jack Patterson, who was later jailed for manslaughter. Such interventions, the report suggests, could have led to better support and potentially averted the tragedy. The findings underscore a critical need for improved responsiveness to housing requests, needs, and risk factors affecting Slough residents.
In its conclusions, the report strongly advocates for greater governmental investment in prevention and early support services, particularly within youth work, to prevent similar devastating outcomes. The case highlights systemic failures in recognising and responding to the severe risks faced by vulnerable young people within the community.
