A plastic packaging manufacturer, Reflex Flexible Packaging Limited, has been fined £60,000 after a worker's hand was crushed in a laminator machine. The Health and Safety Executive (HSE) stated the Mansfield-based company was fined on Monday following the incident.
The employee, a laminator operative and shift supervisor, sustained a severe crush injury while cleaning the machine at the company's Halesfield, Telford site on 17 January 2025. Their left hand was drawn into the machine up to the wrist after a rag became caught between two rollers.
The worker spent four nights in hospital, undergoing two operations and later requiring physiotherapy. An HSE investigation found the company failed to ensure effective measures were in place to prevent access to dangerous moving parts of the machine during cleaning operations.
Inspector Zach Morris from the HSE described the incident as "entirely preventable". Reflex Flexible Packaging Limited previously received a £277,500 fine in November 2025 after an employee died when trapped in an unguarded machine at their Langley Mill, Derbyshire site.
