Synthetic drug cocktails deepen US overdose crisis

The US is experiencing a deepening drug crisis driven by potent, homemade synthetic opioids often mixed with animal tranquillisers, making overdoses harder to reverse.

  • Synthetic opioids like fentanyl are frequently mixed with animal tranquillisers such as xylazine and medetomidine.
  • These mixtures can prolong the high but slow breathing, and naloxone, an opioid overdose reversal drug, is ineffective against the tranquillisers.
  • Overdose deaths in the US peaked at 110,000 in 2022, falling to around 70,000 last year, but the crisis has shifted with increasing stimulant-related fatalities.

A new wave of homemade, synthetic drugs is exacerbating the drug crisis in the United States. These potent cocktails often combine synthetic opioids, such as fentanyl, with animal tranquillisers like xylazine and medetomidine.

Fentanyl, which is entirely man-made, is 50 to 100 times more powerful than heroin. The addition of synthetic tranquillisers makes reviving overdose victims more difficult, as naloxone, a drug that reverses opioid overdoses, does not work against them. Xylazine can also cause deep necrotic skin ulcers.

While US overdose deaths decreased from a peak of 110,000 in 2022 to approximately 70,000 last year, the crisis has shifted. In Franklin County, Ohio, 78% of fatal overdoses in the last 12 months were from stimulants. Many stimulant users are unknowingly consuming crack cocaine mixed with fentanyl or other opioid cocktails, leading to unexpected overdoses.

Why this matters: The mixing of synthetic opioids with animal tranquillisers creates more dangerous drug combinations that are resistant to standard overdose reversal treatments, posing a significant challenge to public health efforts.
