Despite these omissions, Cregger's interpretation is described as one of the most accurate and interesting video game films made. It focuses on recreating the experience of playing the game, with a plot centred on a zombifying virus released in Raccoon City by the Umbrella Corporation.

The film follows Bryan Hodukavich, a courier thrust into a viral outbreak without weapons or knowledge of events. He navigates locations reminiscent of the games, finding weapons and ammunition, while his understanding of the world is shaped by found footage and enigmatic interactions with non-player characters.

Cregger has explained his vision in a video, stating his aim to reflect not just the fictitious universe of the games, but also the player's experience of moving through an evolving horror world. Theories have also emerged regarding Bryan's role as a precursor NPC, leaving hints and ammo for a future lead character.
